2003/2004 Registration Schedule

Monday-Friday
August 4-8 from 11:00AM-2:00PM
August 11-22 from 9:00AM-3:00PM
No registrations will be taken on the first day of school (August 23, 2003)

To Register Your Child For The 2003-2004 School Year, You will Need:

1. Child’s Birth Certificate
2. Social Security Card
3. Immunizations and Physical - the minimum requirement is: 4 DPTs, 3 oral polios, and 2 MMRs. All shot records need to be signed or stamped with the physician’s signature
4. Proof of Residency - Must be a legal document such as settlement papers, Deed of Property, current Rental or Lease Agreement per Board of Education Policy No. 5111
5. Only Parent or Legal Guardian may register

Additional requirements for Kindergarten Registration

1. All incoming kindergarten students are required to have a current physical within six months of September, 2003
2. Child must be 5 yrs old by November 30, 2003
